I have just discovered s form on one of my sites has gone. The module is still there as is a submit button with no text in it. It was a contact form containing some conditional fields. When i go to edit the form there are no fields in the form at all. The whole thing has just gone blank.

I'm using Live Forms 5.8.3 and DNN 9.04.04

Has anyone had this happen and been able to recover the form? The module is in use on another page with a different form and seems to be working OK.
