Raising Tickets

In the attachments below, Users can create tickets but they don't reach the Unassigned issues. This means that we must go through each form to see if any tickets have been raised and it also means that ticket can't be responded to or closed (only deleted). 

The tickets are also not reaching our SQL database due to this issue so our reporting is off. What may be causing this?

Would we be able to organise a time slot as this a small issue which is severely affecting our ticketing system. 

How much would it cost to look into?
