Exception - Mandeeps.DNN.Modules.LiveForms.Components.Conditions.EqualsTo.HasMetCondition

I was curious if anyone has any idea on how to track down what might be causing this exception?  I am guessing from the trace that there is a condition that is failing however I am not able to find out which field is causing it (would be very helpful if it was included in the exception details).

Note: I am using LiveForms 5.9.19

I ended up recreating the form to fix it.  That said, I have a backup of the database -- can I extract it from there?

Please upgrade the Live Forms to its latest version which is v6.0.8. You can download the latest version of our products from https://www.mandeeps.com/downloads
